Age Limit: Candidates applying for MIS cum Field coordinator, Area Hospital Accountant DEO, SNCU and DEO, IDSP should have below 35 years age as on dt. 05-02-2013.Age relaxations will be extended as per rules. Qualification: Candidate must possess General Nursing and Midwifery for Sr. No. 1, 2 posts, H.S.C Examination for Sr. No. 3, 10, 11 posts, + 2 Science D.M.L.T from AICTE for Sr. No. 4, 5 posts, Commerce graduate/ Any Graduate having PGDCA/DCA/MCA/BCA for Sr. No. 6, 7, 8, 9 posts, MBBS Degree for Sr. No. 12 posts & D.Pharma for Sr. No. 13 posts. Prescribed qualifications from recognized universities/ Institutions are only accepted. |
1. Two copies of passport size colour attested photograph to be submitted along with the application. 2. Two self addressed envelope size 4.5”x9.5” (with registered postage stamp) 2. The following documents are to be enclosed along with the application: a) Recent Residential certificate issued by the Tehsildar/ Sub-Collector etc. (duly attested) b) Attested photo copies of all mark sheets/ certificates in proof of the claim made by the candidate relating to his educational qualification. c) Experience certificate if any. d) Work certificate, date of joining & date of retrenchment from the Project (in case of candidates who are availing benefits of previous projects etc.) e) The Candidate applied for MIS cum Field coordinator, Area Hospital Accountant DEO, SNCU and DEO, IDSP should have below 35 years age as on dt. 05/02/2013. f) Cast certificate issued from competent authority. f) Valid Id Card form District Employment Exchange. |

Fill the application, affix Two copies of passport size color attested photographs, 02 self addressed envelope size 4.5”x9.5” (with registered postage stamp) and attested copies of all relevant documents in supporting to age, caste, residence, mark sheets/certificates and all other documents must be sent by super scribing the name of the post to be applied for on the envelope to CDMO, Jharsuguda, AT/PO/Dist- Jharsuguda on or before 18-02-2013.
